Well Certificates

If a conveyance document requires a Certificate of Real Estate Value at the time of recording, it will also need a Well Disclosure Statement.

If there are no wells on the property, the instrument must contain a statement attesting to this fact. If there is a well or wells located on the property, a completed Well Disclosure Statement must accompany the document.

The Well Disclosure Certificate form and additional information is available on the Minnesota Department of Health Well Management website.

E-Well Disclosure Certificate

The disclosure certificate is now available from the Minnesota Department of Health (MDH). Effective July 1, 2009, the Minnesota Department of Health, Well Management Section, is now accepting Well Disclosure Certificates filed electronically.

This service is one part of the framework for electronic real estate recording and reflects nearly two years of effort by county recorders and MDH staff in the design, development, and testing of e-Well Disclosure Certificate (e-WDC).
